Background
KCSON, with funding from Baylor‑Uganda, implemented this project in Kyenjojo District to promote bi‑directional clinical and community referrals, strengthen linkages, and improve HIV service delivery using the Community Facility Framework. Working in four sub‑counties — Bugaaki, Butiiti, Nyabuharwa, and Butunduzi — the project targeted eight health facilities serving as referral centers for clients identified in communities.
Fourteen Community Health Workers were engaged to track referrals and routinely report to their attached facilities and KCSON, supporting progress toward the 95‑95‑95 HIV targets.
